In this vital role, you will assist with daily living activities, administer medications under supervision, and support the overall well-being of individuals across various care settings. Responsibilities Provide direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Administer medications accurately according to prescribed orders, ensuring compliance with all safety protocols and documentation standards. Monitor vital signs and observe patients for any changes in condition, reporting findings promptly to nursing staff. Maintain detailed medical records and documentation within electronic health record (EHR) systems such as Point Click Care. Support specialized care needs including dementia, Alzheimer's, memory care, behavioral health, and hospice care by implementing individualized care plans. Assist with medical procedures such as catheter care, phlebotomy (blood draws), and aseptic techniques during treatments. Collaborate with multidisciplinary teams in diverse settings. Skills Proven experience working with individuals with disabilities or in senior care environments such as nursing homes or assisted living facilities. Familiarity with EMR/EHR systems including Point Click Care, Matrix, or similar platforms for medical documentation. Strong knowledge of medical terminology, physiology, anatomy, and vital signs monitoring. Ability to perform medication administration accurately while adhering to safety protocols and HIPAA guidelines. Competence in using patient handling equipment like Hoyer lifts and understanding of geriatrics and dementia-specific care strategies. Excellent caregiving skills combined with a proactive attitude toward patient service and teamwork across various healthcare disciplines including pediatric (pediatrics) and behavioral health settings.
