Overview Join our dedicated healthcare team as a Day LVN/RN working from 6 a.m. to 6 p.m., where your expertise will make a meaningful difference in patient care. You will be instrumental in delivering high-quality, compassionate care while collaborating with multidisciplinary teams to ensure optimal patient outcomes. Responsibilities Perform detailed patient assessments, including vital signs, physical examinations, and comprehensive health histories. Administer medications and injections accurately, including IV infusions and catheter care, adhering to HIPAA regulations and safety protocols. Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to document patient information meticulously and ensure seamless medical documentation. Conduct diagnostic evaluations, coordinate with physicians for treatment plans, and assist with procedures like wound suturing or spinal taps. Manage airway support and ventilator care for patients requiring respiratory assistance in various settings such as ICU or emergency rooms. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions within memory or senior care facilities. Collaborate with case management teams to develop discharge plans, coordinate post-acute care, and facilitate patient education on health coaching and disease management strategies. Experience Valid LVN or RN license with current state certification required Demonstrated proficiency with EMR/EHR systems Critical care experience including Level I Trauma Center or Level II Trauma Center environments is highly desirable. Experience working with individuals with developmental disabilities or in assisted living and long-term care facilities is a significant plus. Background in acute pain management, infectious disease care, hospice services, or Strong assessment skills combined with physiology knowledge and the ability to perform venipuncture, phlebotomy, tube feeding, and sterile procedures confidently.
