Job Description

Hospice PRN – Registered Nurse (RN) Location: Lubbock, TX 79414 Agency: High Plains Senior Care Hospice Employment Type: Full-Time Job Summary High Plains Senior Care Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to provide high-quality hospice care to patients in their homes. The RN will perform comprehensive patient assessments, coordinate care with the interdisciplinary team, educate patients and families, and ensure comfort-focused, patient-centered care throughout the hospice journey. Key Responsibilities Perform initial and ongoing comprehensive patient assessments. Develop, implement, and update individualized plans of care. Provide skilled nursing care and symptom management for hospice patients. Educate patients and their families on disease progression, medications, self-care, and end-of-life care. Coordinate care with physicians and the interdisciplinary hospice team. Monitor patient condition and communicate changes to the physician and care team. Complete accurate and timely clinical documentation and progress notes. Assess caregiver needs and provide appropriate education and support. Make referrals to appropriate community resources and support services as needed. Participate in case conferences and collaborate with the interdisciplinary team. Provide on-call coverage as assigned. Follow all agency policies, state regulations, and hospice standards of care. Qualifications Current Registered Nurse (RN) license in the State of Texas. Minimum of 1 year of RN experience required. Home Health and/or Hospice experience preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and collaboratively within an interdisciplinary team. Valid driver's license, reliable transportation, and current auto insurance.
