We're growing! Peaceful Hearts Hospice is seeking a compassionate, organized Registered Nurse (RN) Case Manager to join our team. As an RN Case Manager, you'll provide exceptional end-of-life care while coordinating services for patients and their families across a variety of care settings. What You'll Do Manage a hospice patient caseload from admission through discharge. Complete comprehensive nursing assessments and develop individualized care plans. Provide skilled nursing care, including symptom management, wound care, medication education, and end-of-life support. Coordinate care with physicians and the interdisciplinary hospice team. Educate and support patients, families, and caregivers throughout the hospice journey. Complete accurate and timely electronic documentation. Supervise Hospice Aides and Licensed Practical Nurses in accordance with hospice regulations. Participate in an on-call rotation to support patients and families after hours. Provide care in homes, assisted living communities, skilled nursing facilities, and other residential settings. Qualifications Current Nebraska Registered Nurse (RN) license in good standing. Valid driver's license, reliable transportation, and current auto insurance. Hospice, home health, oncology, or related nursing experience preferred.
