Coordinate with physicians and other healthcare providers for diagnostic evaluations, laboratory specimen collection/processing, and follow-up care. Ensure compliance with all long-term care regulations and safety standards across diverse settings including skilled nursing facilities and assisted living communities. Experience Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice. Prior experience working in home health environments or community-based settings is highly preferred.
