Licensed Practical Nurse (LPN) – Part-Time, and PRN Crystal Cove Post Acute – Lacey, WA Pay: $38–$45/hour DOE Shifts: 12hr Rotating Schedule About Crystal Cove Post Acute Crystal Cove Post Acute is a licensed skilled nursing facility located in Lacey, Washington, providing high-quality post-acute, rehabilitative, and long-term care. Position Summary We are seeking Licensed Practical Nurses (LPNs) who are compassionate, detail-oriented, and dedicated to providing resident-centered care in accordance with federal, state, and facility standards. The LPN works under the direction of the RN and DON to ensure regulatory compliance, quality outcomes, and adherence to individualized care plans. Key Responsibilities Deliver nursing care consistent with LPN scope of practice. Administer medications and treatments per physician orders, documenting accurately in the EHR. Monitor resident conditions and promptly report changes to the RN or provider. Participate in care planning, interdisciplinary meetings, and QAPI initiatives. Maintain infection control and safety protocols consistent with CDC and DOH guidance. Support CNAs and ensure daily care routines align with each resident’s care plan. Uphold residents’ rights, dignity, and confidentiality at all times. Qualifications Active Washington State LPN license in good standing. Current CPR/BLS certification (or willingness to obtain). Familiarity with long-term care regulations (CMS, DOH, and OSHA). Strong communication, organization, and teamwork skills.
