Licensed Vocational Nurse (LVN) – PM Shift (4-2 Rotation)

Yucaipa Hills Post Acute (formerly Calimesa Post-Acute) is an 82-bed skilled nursing facility seeking compassionate and dedicated LVNs for our PM shift. Our mission is to provide exceptional 24-hour care while supporting and valuing our staff.

Position Details

Shift: PM (3:00 PM – 11:30 PM)

Schedule: 4-2 Rotation

Job Type: Full-time

Pay: Based on experience

Key Responsibilities

Administer medications and treatments per physician orders and facility protocols.

Observe, document, and report patient responses to medications and treatments

Perform patient assessments and contribute to care planning

Maintain a safe, clean, and comfortable environment for patients and staff

Follow infection control protocols and promote patient hygiene

Implement and evaluate individualized care plans

Assist with discharge planning, including treatments and physician orders

Maintain a clean, safe, and comfortable environment for patients and staff

Monitor and report changes in patient condition

Requirements

Previous experience in a patient care setting preferred

Strong understanding of the nursing process (assessment, planning, implementation, evaluation)

Active LVN/LPN license in the State of California

Current BLS (Basic Life Support) certification
