Job Summary Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) specializing in Assisted Living and Memory Care. In this vital role, you will provide compassionate, skilled nursing care to residents with a focus on dementia, Alzheimer's, and other memory-related conditions. Your expertise will support residents' well-being, promote independence, and enhance their quality of life. Responsibilities Deliver personalized nursing care to residents with memory impairments, ensuring safety and comfort at all times Administer medications and treatments accurately, including IV infusions and injections, following established protocols Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform care plans Manage complex care needs such as acute pain management, catheterization, tube feeding, and ventilator support when necessary Document medical records meticulously using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations Collaborate with interdisciplinary teams to develop discharge planning, case management strategies, and individualized care plans Provide education and support to residents’ families about dementia care, medication administration, and safety procedures Experience Valid LPN license with current state licensure required Prior experience working in assisted living or memory care environments strongly preferred Demonstrated knowledge of Alzheimer’s disease, dementia care, and behavioral health management Experience with EMR system Point Click Care Background in long-term care settings including skilled nursing facilities or hospice care preferred Familiarity with IV infusion techniques, medication administration protocols, and acute pain management practices Knowledge of infection control policies, HIPAA regulations, and patient confidentiality standards Join us to be part of a vibrant team committed to enriching the lives of our senior residents through compassionate and expert nursing care.
