Position Summary As a Home Health LPN, you will work closely with patients, families, and the interdisciplinary care team to deliver skilled nursing services and support individualized care plans. This float position serves patients throughout Central District and North King County. Responsibilities Visit patients in their homes and provide skilled nursing care according to the established plan of care. Implement physician-ordered and clinician-directed care plans. Monitor and evaluate the effectiveness of patient care and report changes in condition. Accurately document patient assessments, treatments, and medical history. Provide emotional support, education, and encouragement to patients and their families. Complete and submit timely progress notes and visit documentation daily. Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure quality patient outcomes. Promote patient safety, comfort, and independence. Knowledge and Skills: Knowledge of infection control policies and procedures, i.e., Standard Precautions, Isolation Precautions, personal protective equipment, hand hygiene. Ability to work in an interdisciplinary team, supervised by a nurse Knowledge of the delivery of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
