Lubbock's fastest growing and top chronic care management company currently has an opening in our Lubbock office for a LVN Care Manager and is offering a $2,500 sign on bonus!!
This is a full-time position and responsibilities include, but are not limited to:
Contacting patients by phone to discuss their current medical condition(s) and needs
Creating care plans
Working directly with physician's offices
Overseeing a team of Medical Assistants
Advance Care Management's ideal candidate is a team-player, punctual, and enjoys talking one-on-one with patients about their medical needs. Must be computer proficient. In addition to our base pay we offer:
$2,500 sign-on bonus
Monthly goal bonus
Quarterly retention bonus
Additional preference will be given to applicants who have:
Experience in a clinic or MD office;
Experience managing or overseeing staff
