What We Offer $28.00–$30.00/hour Monday–Friday office Schedule Weekends Off Paid Orientation No Home Health Experience Required – We Will Train Supportive Team Environment Stable Full-Time Position Qualifications Active California Licensed Vocational Nurse (LVN) license required Bilingual in English & Spanish (Required) Minimum 1 year of hands-on clinical nursing experience (school clinicals do not count) No Home Health or Case Management experience required – we are willing to train the right candidate Strong communication, organizational, and multitasking skills Basic computer proficiency Valid California Driver's License and reliable transportation Job Responsibilities As a Home Health In Office Case Manager, you will: Coordinate patient care with physicians, nurses, therapists, and other healthcare professionals. Review referrals and assist with patient admissions. Develop and coordinate individualized plans of care. Communicate professionally with physicians regarding patient care and orders. Coordinate nursing and therapy services to ensure timely patient care. Review clinical documentation for accuracy, completeness, and regulatory compliance. Participate in interdisciplinary care conferences. Assist with discharge planning and coordination of community resources. Maintain patient confidentiality and accurate documentation. Educate and support patients, families, and clinical staff. Participate in quality assurance activities and chart audits. Collaborate with the clinical team to provide exceptional patient care.
