Per Diem Registered Nurse (RN) – Home Health in Yucaipa, CA

Position Overview Legacy Care Home Health is seeking an experienced and reliable Per Diem Registered Nurse (RN) with home health experience to provide high-quality patient care in the home setting. This role is ideal for a self-motivated nurse who is comfortable working independently and delivering compassionate, patient-centered care. Responsibilities Provide skilled nursing care to patients in their homes Perform comprehensive patient assessments and develop care plans Administer medications and treatments as prescribed Perform IV therapy and infusion services (preferred) Monitor patient progress and report changes to physician and care team Educate patients and caregivers on disease management and care plans Complete accurate and timely documentation Requirements Active California Registered Nurse (RN) license in good standing Minimum 1 year of home health experience (required) Current BLS certification Valid driver’s license and reliable transportation Strong clinical assessment and critical thinking skills Ability to work independently and manage time effectively Excellent communication and documentation skills Preferred Qualifications Previous experience with electronic charting systems. Pay: $50.00 - $55.00 per hour Work Location: On the road
