Job Summary
Heartland Home Health & Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a PRN Home Health Admission Nurse. This position is responsible for completing Start of Care (SOC) admissions for home health patients in their homes.
The Admission Nurse performs comprehensive assessments, completes OASIS documentation, establishes the initial plan of care, provides patient and caregiver education, and coordinates with the physician and clinical team to ensure a smooth transition into home health services.
This position offers a flexible schedule and is ideal for nurses looking to supplement their income while making a meaningful impact in patients’ lives.
Essential Job Responsibilities
Complete Start of Care (SOC) admissions in patients’ homes.
Perform comprehensive nursing assessments and obtain patient history.
Complete accurate and timely OASIS documentation.
Develop and initiate the individualized plan of care.
Obtain and process physician orders as needed.
Educate patients and caregivers regarding diagnoses, medications, disease management, and home health services.
Coordinate care with physicians, therapists, office staff, and other members of the interdisciplinary team.
Recognize changes in patient condition and communicate appropriately.
Complete all documentation in accordance with Medicare regulations and agency policies.
Maintain patient confidentiality and professional standards of nursing practice.
Qualifications
Current Registered Nurse (RN) license in the State of South Dakota.
Minimum of one year of nursing experience required.
Home health experience preferred.
OASIS documentation experience preferred.
Strong assessment, critical thinking, and communication skills.
Ability to work independently and manage time effectively.
Valid driver’s license, reliable transportation, and current auto insurance.
Compensation
$130 per completed Start of Care (SOC) Admission
Flat-rate compensation (no mileage reimbursement)
Flexible PRN scheduling
