RN Case Manager Hospice Job Summary: Shalom Hospice of Washington is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Case Manager in and around Kitsap County. As a RN Case Manager, you will plan, organize, and direct hospice care for terminally ill patients and their families, providing physical, emotional, and spiritual comfort and support. Essential Job Functions/Responsibilities: Patient Care: Completes comprehensive assessments of patient and family to determine hospice needs Provides professional nursing care utilizing the nursing process Initiates and revises plan of care as patient status and needs change Communication: Maintains accurate and relevant clinical notes regarding patient's condition and care given Communicates with physicians, community health related persons, and families/caregivers Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency Additional Duties: Participates in on-call duties as defined by the on-call policy Supervises ancillary personnel and delegates responsibilities when required Fulfills the obligation of requested and/or accepted case assignments Position Qualifications: Graduate of an accredited school of nursing Registered Nurse, with a License in good standing to practice Minimum of one (1) year of experience as a professional nurse within the last three (3) years or a baccalaureate degree in nursing and minimum of two (2) years' experience Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, and nursing skills per competency checklist Ability to lift, position, and/or transfer patients, and to lift supplies and equipment Possesses and maintains CPR certification What We Offer: Competitive hourly rate: $104,000- $114,400 Annual Salary with a $7,000 Sign-On Bonus available for this specific position if job started on or before September 1st, 2026.
