Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN/LVN) for a 13-week travel assignment in Chamberlain, South Dakota. The LPN will provide compassionate nursing care to residents in a Long-Term Care/Skilled Nursing Facility while collaborating with the interdisciplinary healthcare team to ensure quality resident care.
Responsibilities
Provide direct nursing care to residents in a long-term care setting.
Administer medications and treatments as prescribed.
Monitor and document residents' conditions and report changes to the RN or physician.
Assist with patient assessments and care planning.
Support residents with activities of daily living (ADLs).
Maintain accurate documentation using the electronic medical record.
Follow infection prevention and medication safety protocols.
Collaborate with physicians, nurses, therapists, and other healthcare professionals.
Provide resident and family education.
Maintain a safe, clean, and compassionate care environment.
Requirements
Active Licensed Practical Nurse (LPN/LVN) license.
Minimum 1 year of Long-Term Care or Skilled Nursing experience.
Current BLS Certification.
Experience with MatrixCare EMR preferred.
Strong communication, teamwork, and organizational skills.
Ability to work 12-hour night shifts, including weekends and holidays.
