Job Description

Licensed Vocational Nurse (LVN) will deliver direct nursing care to residents across multiple group homes in Lubbock, Texas. The 26‑week assignment focuses on supporting medically fragile individuals with IDD, behavioral challenges, and mental health needs. Key Responsibilities: Deliver direct nursing care to residents across multiple group homes, supporting medically fragile individuals with IDD, behavioral challenges, and mental health needs. Monitor and maintain enteral nutrition support, including g‑tube care and enteral feedings, ensuring proper administration and resident tolerance. Perform urinary catheter care, including Foley insertion support, routine care, monitoring output, and documenting findings per protocol. Execute tracheostomy care and respiratory status monitoring, promptly identifying changes in condition and escalating concerns to the appropriate clinical team. Document assessments, medication administration, treatments, and incident reporting accurately and timely, collaborating with interdisciplinary staff to ensure continuity of care. Work evenings (2 p‑10 p) and alternate weekends as scheduled, and float or adjust shifts outside preferred schedule with advance notice. Communicate effectively with interdisciplinary teams and maintain thorough documentation. Qualifications: Graduate of an accredited Licensed Vocational Nurse (LVN) program. Active Texas LVN license in good standing (or ability to obtain prior to start). Current BLS (Basic Life Support) certification. Minimum 2 years of LVN experience (non‑negotiable). Prior experience in long‑term care, group home, hospital, and/or corrections settings. Hands‑on competency with g‑tube/enteral feedings, Foley catheter care, and tracheostomy care. Ability to work evenings (2 p‑10 p) and alternate weekends as scheduled. Willingness to float or adjust shifts outside preferred schedule with advance notice.
