Job Description

The LVN Telephone Triage Nurse is responsible for providing telephone triage and basic patient assessment to assist with determining appropriate patient care needs under the direction of the physician and in accordance with established protocols. HOURS: Full-time; Monday–Friday, 8:00 a.m.–5:00 p.m. (Hours may vary depending on business needs.) Location: Must reside in the state of California. JOB RESPONSIBILITIES: Speaks with patients over the telephone to obtain information regarding their medical concerns and assists with determining appropriate next steps under established protocols and physician direction. Performs basic assessment and data collection regarding the patient’s condition, including obtaining information about the chief complaint and, when available, vital signs and mental status. Communicates patient information and assessment findings to the physician or other appropriate licensed healthcare professional and follows established protocols for escalation of care. Assists in identifying patients who may require urgent or emergency care and promptly escalates concerns to the physician or appropriate licensed healthcare professional. Schedules appointments for patients who require an office visit or coordinates with an appointment scheduler to assist the patient. Documents patient calls, assessments, recommendations, and follow-up in the electronic medical record in accordance with established policies and procedures. Performs other duties as required by the position. EDUCATION & EXPERIENCE REQUIRED: High school diploma or equivalent required. Graduation from an approved vocational nursing program required. Current, active California LVN license required. Six months to one year of LVN experience preferred, but not required. Clinic experience preferred, but not required. Strong interpersonal and communication skills, with the ability to maintain effective working relationships with coworkers and communicate effectively with diverse patient populations. SKILLS & QUALIFICATIONS: Knowledge of medical terminology, anatomy, physiology, and pathophysiology. Familiarity with healthcare systems, regulations, policies, procedures, and documentation standards. Knowledge of infectious disease management and control practices and safety standards. Ability to recognize and promptly escalate urgent or emergent patient concerns. Ability to follow established telephone triage protocols and physician-directed procedures. Ability to perform mathematical calculations for medication dosages. Ability to effectively manage and prioritize multiple tasks simultaneously. Strong verbal and written communication skills. Ability to maintain confidentiality and exercise sound judgment when handling sensitive patient information.
