Job Description

Licensed Practical Nurse (LPN) – Long-Term Care (LTC)

Schedule: Full-time PM shift, 2:00 PM – 10:00 PM Weekend Requirement: Every other weekend Contract Length: 13 weeks Location: New Lisbon, WI

Position Overview

We are seeking a Licensed Practical Nurse (LPN) for a 13-week contract opportunity in a long-term care setting. The LPN will provide resident care, administer medications, monitor changes in condition, communicate with physicians and family members, and assist nursing staff as needed.

Responsibilities

  • Administer medications according to the five rights of medication administration.
  • Monitor residents and report changes in condition to the charge nurse.
  • Notify physicians and family members of changes in resident condition as needed.
  • Report any suspected abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
  • Assist CNAs with call lights and other resident care responsibilities.
  • Perform additional duties as assigned.

    Requirements

  • Active Wisconsin LPN license.
  • Current BLS/CPR certification.
  • Long-term care experience preferred.
  • Strong attention to detail and medication administration accuracy.
  • Ability to provide holiday coverage as needed.

    Additional Information

  • Full-time afternoon shift, 2:00 PM – 10:00 PM.
  • Every other weekend required.
  • 13-week contract.
  • Meditech EMR experience.

    Pay: From $40.00 per hour

    Expected hours: No less than 40.0 per week
