Overview We are seeking a dedicated Home Health Nurse to provide comprehensive nursing care to clients in their homes. The ideal candidate will possess a strong clinical background, including critical care experience and proficiency with electronic health record systems. This role offers an opportunity to make a meaningful difference by delivering high-quality patient-centered care across diverse settings. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations. Administer medications, injections, and perform catheterizations. Manage complex cases involving acute pain, dementia, Alzheimer's disease, and infectious diseases while coordinating with interdisciplinary teams. Provide specialized care such as wound management, tube feeding, ventilator support, and post-acute care procedures. Document all patient interactions accurately using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems in compliance with HIPAA regulations. Educate patients and their families on health coaching, medication administration, and disease management strategies. Support patients with developmental disabilities or those requiring memory or behavioral health management in home and community settings. Experience Proven nursing experience with a background in critical care environments. Demonstrated expertise in home health care, hospice, or post-acute settings with familiarity in ICD-9/ICD-10 coding for documentation accuracy. Experience working with diverse populations including geriatrics, individuals with disabilities, or those requiring dementia or Alzheimer's care. Knowledge of infection control protocols and sterile processing techniques essential for safe patient care delivery. Strong understanding of anatomy and physiology principles to inform accurate patient assessments and diagnostic evaluations.
