The ideal candidate is detail-oriented, collaborative, and committed to delivering exceptional resident care. Key Responsibilities Coordinate and complete accurate MDS assessments in accordance with CMS guidelines. Develop and update individualized resident care plans. Ensure timely MDS submissions and reimbursement accuracy. Collaborate with physicians, therapy, nursing, and interdisciplinary teams. Monitor regulatory compliance and prepare for state surveys. Audit documentation to ensure accuracy and completeness. Educate staff regarding documentation requirements and quality standards. Track quality measures and identify opportunities for process improvement. Provide nursing floor support as needed to maintain quality resident care. Participate in resident assessments, care conferences, and discharge planning.
